Extra Mucosal Cardiomyotomy (Heller's myotomy) remains the most common surgical treatemnt for achalasia of cardia. Twenty eight patients udnerwent transthoracic heller's myotomy for achalasia of cardia at PGMI LRH, Peshawar from May 1990 to April 1995. No antirelfex procedure was combined with the myotomy. there were no hospital deaths. One patients had a reoperation for a leak. All patients have excellent to good clinical results with no gastroesophageal refllux. The follow up ranges from 12 months to 73 months with a mean age 34.5. We conclude that a properly performed Heller's Myotomy can give excellent results in the management of esophageal achalasia, without an antireflux procedure.
